Connie, Connecticut’s Statewide Health Information Exchange

The state of Connecticut requires licensed healthcare providers with an electronic medical record to participate in “Connie,” the statewide Health Information Exchange (HIE). Connie provides your healthcare providers with a complete view of your medical information which allows for quicker and safe visits and saves you the costs of unnecessary care.

What does this mean for our patients?

Yale Medicine Patients:

Unless you opt-out, health information related to your visits at a Yale Medicine facility will be sent to Connie. The information required to be shared with Connie includes the purpose of your visit, diagnoses, lab results, imaging results, treatment information, medications, health concerns, and medical history.  

Yale Health Patients:

Due to Yale Health’s population of both members and students and consideration for the additional protections provided under FERPA, we are currently not sharing information with Connie. Yale Health members and students who receive care elsewhere in the Yale New Haven Health System should expect information from those visits to be shared with Connie as requirement by law.

How can I opt-out of Connie?

You may opt-out and disable the transmission of your health information to Connie by calling 1.866.987.5514 or completing and submitting an Opt-Out form to Connie by mail, fax, or through their website at https://connect.conniect.org/OptoutForm.
